New Facilitator Application: Facilitator Approval Form

This reference form must be completed by the Elijah House Facilitator in charge of the Facilitated School in which the applicant completed their training.

Directions: The applicant you have listed is applying to become a Facilitator for the Elijah House Schools of Prayer Ministry. We would appreciate your candid assessment of this individual's potential for facilitating a school. Your comments about this individual's spiritual maturity, gifting, motivation, and initiative are necessary to evaluate the applicant's qualifications for conducting a school. Your comments are very important to us and will be kept private for internal Elijah House staff use only.
